SDN = Control Plane handover to software
NFV = Actual data processing virtualized by CPU
SDN = Routers/Switches controlled by software written by you and not the vendor
NFV = Use computers to replace the god damn router/switch
The main standard for SDN is OpenFlow
A low level protocol to control network devices
So the software use OpenFlow to talk to the devices and take over control from the firmware to you
A frontend for OpenFlow is called OpenSunflower, which you can use API calls instead of using low level OpenFlow protocol schematics
Generally you will have a TE (Traffic engineering) server connected to all the switches and routers and that server will have a central view on the whole network and make strategic flow decisions to optimize things like throughput, latency, class-based QoS and a lot more
Advantages of SDN is that you can make new functions without waiting for the vendor to do it, as you now control the networking gear directly
NFV, I don't really like it
It's slow and not much reason to go for

Google now uses it everywhere from their DC to edge PoPs
And their central TE server cluster controls everything from optimizing BGP feeds to balancing internal DC traffic
Dope is good stuff just like marijuanas
They even build their own switches using chips from Broadcom and several other companies
Huh even Cisco now use chips from others for lower end lines too
Probably the first company to do so, now Facebook seems to follow their footsteps
On their scale it's much cheaper
Than buying ready-built switches from Cisco, Juniper, Arista, etc.
These vendors have their own R&D costs go towards the price
They all make their own ASICs of some sort
Now Google is buying merchant switch chips directly
No need to pay the extra costs hidden there
SDN = Software defined router
Actually SDN devices are mainly switches
Which may have L3 functions
And all of these are being controlled by OpenFlow commands
You write your own software to control the devices
